Person centered supports for adults with developmental disabilities who choose to live independently in their own homes. J3 provides services in alignment with Title 17 regulations and the principles of the Lanterman Act, ensuring choice, dignity, and individualized support.

Personal care routines including bathing, dressing, grooming, hygiene, and support with morning and evening routines.
Meal preparation such as planning simple menus, cooking meals, assisting with safe food handling, and helping with grocery lists or shopping.
Household management including laundry, light cleaning, organizing living spaces, and maintaining a safe and tidy home environment.
Transportation support to medical appointments, errands, activities, and community events, along with guidance navigating public spaces safely.
Daily structure and routine building to help clients stay consistent, stable, and confident in their day-to-day independence.

Medical appointment support including scheduling, preparing notes or concerns, attending appointments when needed, and helping clients understand provider instructions.
Medication reminders to support safe and consistent use of prescribed medications and health routines.
Follow-up communication with doctors, therapists, nurses, and other care team members to ensure all updates are understood and carried out correctly.
Health monitoring assistance such as observing changes in mood, behavior, or physical condition and reporting concerns promptly
Wellness routines including support with healthy habits, daily check-ins, and reinforcing provider recommendations to promote overall well-being.

Daily living skills such as planning routines, organizing tasks, cooking basics, and maintaining personal responsibilities.
Budgeting support including understanding expenses, managing spending, using simple budgeting tools, and building financial awareness.
Communication skills such as practicing conversations, expressing needs clearly, and improving social confidence.

Locating housing options that match the client’s needs, preferences, accessibility requirements, and budget.
Assistance with applications including gathering documents, completing forms, submitting requests, and tracking application status.
Communication with property managers to clarify expectations, schedule tours, address questions, and support positive relationships.
Move-in coordination including preparing checklists, setting up utilities, organizing essential items, and helping the client settle in safely and comfortably.
Ongoing housing stability support such as reminders for rent, maintenance requests, and problem solving with the housing team when issues arise.

About J3
J3 Supported Living is a family-run organization founded in 2025, but our home care journey began in 2020 in the midst of the COVID pandemic. We started by serving seniors and families through home care organizations licensed under the Department of Social Services. During this period we saw the true impact of quality care and how it could strengthen a person’s well being, protect their independence, and in some cases, truly save lives.
We carry these experiences into our work with the Regional Center of the East Bay and the broader DDS community. The philosophy of Title 17 guides our approach by honoring choice, dignity, and person-centered support. Our goal is to create an environment where each individual can live with confidence, stability, and meaningful independence.
